NHS Discharge Medicines Service: part two

Welcome to the second module in our series looking at the NHS DMS. It is based on the Centre for Pharmacy Postgraduate Education (CPPE) NHS Discharge Medicines Service: improving outcomes for patients transferring care e-learning programme, and is designed to give pharmacy technicians an overview and better understanding of this area of practice.

Learning objectives:

• Be able to describe how the service is delivered by community pharmacy professionals
• Know how the service relates to the principles of medicines optimisation.
